Good afternoon parent carers,

We have been in discussions with the vaccinations inequalities teams in Suffolk as we wanted to ensure that with the Covid-19 vaccination now being offered to vulnerable 5-11 year olds, that Mental Health or SEND wouldn’t leave any children at a disadvantage.

We have devised the survey below and ask if you could please complete it with any feedback that could cause a barrier to access:
